Certified Hospice Aide – Per Diem, On-Call

Care Partners is seeking a per diem on-call Certified Hospice Aide to join their team. As a Certified Hospice Aide you’ll get the opportunity to work with programs and services designed to honor choice and dignity, while providing comprehensive patient and family care. Your responsibilities will be to:

  • Provide patients with personal care and light housekeeping in accordance with the individualized plan of care and visit frequency assigned
  • Support skilled nursing staff across various settings
  • Maximize the comfort, safety and health of patients and families consistent with hospice philosophy and the organization’s policies and procedures
  • Maintain up to date patient records in accordance with policies and procedures so plans, actions, goals and problems are accurately and clearly stated

Knowledge & Skills needed:

  • Demonstrated skill in communication; patient documentation requirements, and service coordination with the ability to work successfully with patients and family members in aspects of terminal care
  • Basic keyboarding and computer operation skills are required
  • Working knowledge of concise, yet thorough clinical documentation of patient assessments and care required

Qualifications:

  • Requires a minimum of two years of prior CNA/Hospice Aide experience, preferably including experience delivering services outside of the hospital setting
  • Requires successful completion of a CNA training program. (Qualified programs must total 75 hours with 16 hours of supervised practical training as required in 42OR 484.36.) CNA II certification preferred
  • Bi-lingual, Spanish speaking ability preferred
  • Certified Hospice and Palliative Nurse Assistant (CHPNA) preferred
  • Must have a valid driver’s license and acceptable driving record and have access to a reliable automobile to travel to and from patient homes
  • Basic Life Support CPR card for Health Care Provider. Valid card must be obtained prior to patient contact and provided within two weeks of hire

Schedule:

On-call and requires ability to provide coverage 7 days a week, as needed.

Revenue Cycle and Accounts Payable Specialist, Full Time

The purpose of this position is to ensure accurate and timely inflow and outflow of funds to Care Partners through the processing of billing; accounts receivable; payroll; accounts payable, and charitable donations.  This position assures compliance with wage and hour laws, organizational policies and generally accepted accounting practices.

This position ensures accurate and timely billing and appropriate, compliant reimbursement for all services rendered by the organization from Medicare, Medicaid, TriCare, HMOs, PPOs, other insurance companies as well as from private individuals.  This position supports and oversees claims submission and revenue collection by outsourced revenue cycle service providers.

Qualifications and experience:

  1. Two years of accounting, accounts payable and/or payroll experience
  2. One-two years of medical billing experience preferred
  3. Associates degree in accounting or business with a minimum of a full year of college accounting
  4. Demonstrated skill in bookkeeping, accounting and financial record keeping
  5. Proficiency in use of Microsoft Excel and Quickbooks required
  6. Must be reliable in meeting exacting or highly accurate work results, strict deadlines, and prompt and friendly service delivery
  7. Must pass a pre-employment background check and drug screen following job offer
  8. Salary is commensurate with experience

Hospice Nurse Position Responsibilities

  • Requires current Registered Nurse (RN) State of Oregon license prior to first day of employment.
  • Prefer one- two years of nursing experience, preferably in hospice, palliative care, home health or medical surgical care.
  • Requires well developed critical thinking, patient assessment and problem solving skills.
  • Requires the ability to work independently and as part of an interdisciplinary team.
  • Requires strong verbal and written communication skills and familiarity with use of an electronic medical record
  • Must have a valid driver’s license and access to a reliable automobile to travel to and from patient homes.

Responsibilities & Duties:

  • Direct and manage the delivery of quality clinical services on a day-to-day basis consistent with agency policy, accreditation requirements, applicable state practice acts and applicable state and federal regulations through coordination of patient and staff schedules, provision of advice to staff and patients and directing utilization of program resources (clinical and non-labor).
  • Collaborate with members of the management team to provide leadership required to fulfill the mission, vision, and goals of the organization.
  • Collaborate with clinical staff, providers and managers to improve operating systems and processes. Facilitate efficient and effective implementation of clinical best practices utilizing staff engagement and consensus building.
  • Analyze operations to evaluate performance of the Hospice program and staff staff in meeting organizational objectives. Identify and improve areas requiring cost reduction, service enhancement, quality and/or efficiency improvement.
  • Participate in and coordinate weekly interdisciplinary group meetings to ensure delivery of efficient, effective and state-of-the-art palliative clinical care.
  • Collaborate with other members of the leadership team to ensure that clinical protocols, utilization review, case management, quality assurance, and compliance programs are implemented consistent with Medicare Conditions of Participation and other regulatory standards. Take initiative in the development, monitoring and continuous improvement of performance metrics.
  • Hire, orient, train, supervise and lead qualified and competent hospice staff through articulating vision, creating line-of-sight to organization goals and values, establishing performance expectations, creating accountability and systematically measuring performance progress, and developing and coaching staff to create alignment.

Qualifications:

  • Must be proficient in typing with accuracy and have working knowledge of word processing and spreadsheet software.
  • Requires excellent interpersonal and customer service skills with the ability to maintain composure while under pressure and remain flexible despite interruptions and/or routine changes.
  • Ability to organize and complete multiple assignments on time with accuracy is required.
  • Requires a minimum of two to three years of progressive management and leadership experience.
  • Requires home health or hospice nursing experience.
  • Requires a bachelor’s degree at minimum, master’s degree is preferred.
  • Requires demonstrated success in leading the work of others and collaborating cross-functionally to meet deadlines, manage projects and human talent, and successfully execute a wide range of large-scale business objectives.
  • Demonstrated ability to focus on uncompromising customer service with a proven record of managing successful hospice and palliative care, clinical and financial operations.
  • Expert knowledge of hospice and palliative care strongly preferred.
