When to Refer

If your patient and/or his/her family is in need of comfort care and the interdisciplinary support that hospice provides and you would not be surprised that the patient is not alive in 6 months, please consider making a referral to hospice.  Our skilled nursing staff and medical director(s) will assist in evaluating patient prognosis to assure that Medicare prognostic requirements are met.

How to Refer

Palliative and hospice referrals can be accomplished by calling our caring staff 24 hours a day at 1-888-968-9565 (Fax: 503-648-1282).

At Care Partners, our experienced interdisciplinary team values your established relationship with your patient and the expertise you provide in their care. We provide the day to day 24 hour care for your patient and become their primary contact at all hours of the day. The care team will provide holistic and expert care in pain and symptom management, advanced care planning, and bereavement support while reflecting the partnership we have with you, their attending physician.  Our staff will maintain close contact with you, providing regular updates and consulting you for coordination of the individualized plan of care. We consider our team an extension of your team with a focus on providing high quality care and support for your patients and families in their place of residence.

Admissions criteria for Hospice Care:

  • Patients reside in the Care Partners service area of metro Portland, including Multnomah, Washington, Clackamas, Columbia, and parts of Yamhill county
  • Patients are diagnosed with an advanced or serious illness with a limited life expectancy of 6 months or less, if the disease runs its normal course, and are ready to forego further curative/life sustaining treatment to focus on comfort, symptom management, quality of life, and supportive care.
  • Medicare/Medicaid patients must meet the Medicare requirements for determining terminal criteria based on diagnosis (ICD9 code). However, expert hospice staff can determine this eligibility for you.
  • Referral from you, the provider, for hospice care

Admission criteria for Palliative Care:

  • Patients reside in the Care Partners service area of metro Portland, including Multnomah, Washington, Clackamas, Columbia, and parts of Yamhill county
  • Patients have Regence Blue Cross/Blue Shield, Health Net, CareOregon or Tuality Health Alliance insurance
  • Patients diagnosed with a chronic medical condition with a life limiting prognosis (life expectancy) of two years or less, if the disease runs its normal course.
  • Patient wishes to continue to receive life sustaining treatment.
