pexels-photo (1)If your patient and/or his/her family is in need of comfort care and the interdisciplinary support that hospice provides and you would not be surprised that the patient is not alive in 6 months, please consider making a referral to hospice.  Our skilled nursing staff and medical director(s) will assist in evaluating patient prognosis to assure that Medicare prognostic requirements are met.

We have expertise. We have 31 years of experience mastering the art of end of life care.  Care is supervised by our team of hospice and palliative care physicians.  Our medical director, a medical oncologist has served our patients for 31 years.

We respond. We admit patients within hours of receiving a referral and routinely provide home visits at the time of patient death.  We provide free informational and assessment visits to patients/families on the day we are contacted.

We are accessible.  We provide direct telephone access to hospice RNs 24/7. We do not use an answering service.  We serve patients in Multnomah, Clackamas, Washington, Columbia and counties.

We show up. We specialize in providing care to patients outside of office hours (i.e. evenings, nights and weekends.)

We deliver. Our pharmacy delivers medications to the patient’s residence 24/7.

We invest. As a not-for-profit, community based hospice our resources go to serving patients and providers.  We provide care to patients regardless of their ability to pay for services.

We provide home based palliative care. We provide palliative care services for patients enrolled in CareOregon and Tuality Health Association.  We also provide short-term home based palliative care for terminally ill patients completing palliative radiation and/or chemo therapy.  We provide seamless transitions between palliative care and hospice at the appropriate time.
